A Privacy Notice
Explains how the information you give to the podiatrist is used and protected. Fiona Stubbs (Naish) is the Data Controller for the personal information you provide to her as your podiatrist. Fiona can be contacted on Tel. 07904453374.
Collecting Data
The following personal information may be collected: Name, address, tel. number, date of birth, occupation, historical and current health, Doctor, additional information provided by you and information you have provided via client surveys. Visitor contacts are tracked on our website for analysis via google analytics.
Purpose of Processing Personal Health Data
For the purpose of providing clinical care. The information allows me to provide you with good health care decisions and makes sure your care is safe and effective. I may also use your records to check quality of care and to aid in the planning and provision of future services (Patient Questionnaire) and help investigate any concerns or complaints you may have about your health care.
Sharing Information
Personal information may be disclosed:
- (i) For the purpose advised and related purposes you would reasonably expect e.g. referral to other Health Care Professionals, G.P. or Nurse, where I have consent by you to do so.
- (ii) As required by law or under exceptional circumstances where permitted under the Privacy Act.
- (iii) In our booking systems I may need to disclose some of your personal information to relevant staff e.g. Receptionists.
- (iv) Business transfers - if the practice is sold or merged I may disclose your information as part of that transaction, only to the extent permitted by law and with your consent.
Access to Personal Information
You have the right to access and receive a copy of the personal information I hold about you, by contacting me by phone on the above tel. number.
Change, restrict, delete
You may also have rights to change, restrict my use of, or delete your personal information. In the case of Health Records these are normally exempt from change and deletion requests.
Object
You can normally object to
- (i) my processing of some of your information based on my legitimate interests
- (ii) receiving marketing messages from me after providing your express consent to receive them.
Complain
If you wish to raise concern about my use of your information (and without prejudice to any other rights you may have) you have the right to do so with the Information Commissioner’s Office (I.C.O) at www.ico.org.uk.
Security
The goal is to protect your personal information which is managed confidentially and securely from collection of information to destruction. It is destroyed appropriately to comply with professional obligations.

I will take all reasonable steps to ensure data I collect, use or disclose is accurate, complete, up to date and obtained directly from individuals, or other reputable sources. This policy may change from time to time and will periodically be reviewed and updated as necessary.
